Join us for a Ribbon Cutting honoring Country Creek Family Physicians. Country Creek Family Physicians is the patient's medical home and is founded on the Patient | Provider relationship. They coordinate all of the patient's medical care to improve patient health. Their mission is to provide a broad range of high-quality health and wellness services focused on the needs of their patient community.
Country Creek Family Physicians are happy to announce that they now Pediatric Visits. Book your children's sick and well visits now. Accepting new patients from newborns to adolescents. Meet Tiffany Gerke CPNP and Dr. Meram.
Gift bags will be provided to all Children following all CDC safety guidelines.
